NOTICE IS HEREBY GIVEN that:

1. Five Star Plumbing, Inc. is in the process of dissolving.

2. Five Star Plumbing, Inc. was filed a Notice of Intent to Dissolve with the Minnesota Secretary of State's Office on June 12, 2012.

3. Claims against Five Star Plumbing, Inc. should be made in written form and presented or mailed to:

4. You have ninety (90) days from the date of this Notice within which to file a claim. If such Notice is not presented or mailed to David Smith within such time, your claim will be barred.
